A program on continuity of care in primary healthcare

The Ministry of Social Affairs and Health together with the Ministry of Finance launched a program to promote continuity of care in primary healthcare. The purpose of the program is to support WBSCs in adopting service delivery models that would improve continuity of care, which to date has been lacking. Examples of models that could improve continuity of care include personal doctor, personal nurse or personal care team model. A steering group has been appointed for the program, with a term from 3 February 2025 to 30 April 2027.
